Job Responsibilities:
- Deliver and coordinate high-quality patient care in compliance with physician instructions.
- Continuously monitor and assess patients’ conditions, reporting any changes to supervisors or physicians.
- Accurately document nursing activities, assessments, and patient responses promptly.
- Participate in creating, implementing, and updating nursing care plans.
- Take appropriate nursing actions based on assessments to achieve desired outcomes.
- Identify changes in patient needs and intervene to minimize risk.
- Adhere to Aveanna policies, state and federal regulations, and the Nurse Practice Act.
- Follow infection control procedures to protect patients and staff in compliance with OSHA standards.
- Educate patients and families about disease processes, self-care techniques, and prevention.
- Maintain professional competencies through continuing education and training.
Minimum Qualifications:
- Graduation from an accredited nursing program.
- Current, unrestricted LPN/LVN license in the state of practice. (Compact licenses must be transferred within 90 days.)
- Valid CPR certification (with hands-on component). Aveanna can assist with CPR certification after hire if necessary.
- TB skin test or equivalent screening within the last 12 months.
- Six months of hands-on nursing experience preferred but not required.
- Reliable transportation to and from patient homes.
Preferred Qualifications
- Six months of recent clinical nursing experience.
- Previous experience in home health care.
Specific Licensing and Experience Requirements:
- South Carolina: At least one year of pediatric nursing experience required.
- California: At least one year of licensed nursing experience required.
- Louisiana: One year of licensed nursing experience required.
- Continuing education as mandated by each state’s nursing board.

- Ability to lift and reposition patients and equipment up to 50 pounds without assistance.

- Strong sense of smell and touch to assist with patient care.
- Capability to respond appropriately in emergency situations at patient homes or during transport.
